Job Summary:

JOB SUMMARY

In depth review of patient history including disease presentation, diagnosis, staging, pre-transplant therapy responses, status at transplant and determination of transplant outcomes including toxicity identification and grading, disease response, survival and quality of life. Monitor all disease or toxicity related evaluations (clinical labs, radiology etc.) whether performed at VUMC or an outside institution.

KEY RESPONSIBILITIES

• Conducts chart review and analysis

• Disease History is established including presentation and diagnosis

• Treatment History is established including details of all therapies and responses

• Pre-transplant status is established including documentation of any clinical conditions representing increased risk or influencing transplant outcome

• Post-transplant disease response is established for patient's lifetime, including documentation of relapses, progressions and all post-transplant therapies and responses.

• Transplantation toxicities are established including? documentation and grading, establish causality of all severe, unusual or life-threatening adverse events and secondary malignancies

• Medical records are reviewed for specific transplant related issues for contract submissions and renewals.

• Determines Disease Staging

• Pre-transplant and post-transplant relapse/progression disease staging is determined according to most current implemented disease specific international standard guidelines.

• Conducts Outcomes analysis

• Utilize Transplant Registry and international standards to establish disease response to transplantation and all other administered therapies

• Recommend appropriate post-transplant re-staging studies to Transplant attendings or referring physicians when necessary to ensure that follow-up testing is comparable to pre-transplant evaluations for comparisons

• Patient follow-up conducted? Contact physician offices, utilize electronic resources and contact patients directly to determine survival

• Quality Assurance

• Documentation of unauthorized protocol deviations as requested.

• Assist in production of reports for presentation to the SCT Program Quality Assurance Board relating to identified Program Quality Indicators.

• Enters patient data into database/systems

• Database entry of results of all of the above into appropriate designated database systems for mandatory submission to transplant registries, use in clinical trials and retrospective analysis to determine treatment safety and efficacy.

• Continuing Education

• Maintain a current understanding of transplantation issues by attending conferences and presentations and reviewing relevant literature

• Present summary of pertinent information to other team members in periodic meetings as assigned.
